Dear All,
Let me start by thanking you Pastor JohnMark for the timely and useful contribution on this scary issue.
I really wonder why we have to get this important issue publicized this late (months after the implementers meeting) in Nigeria and not from any government agency, or the media, but an advocacy group - NHVMAG. Ordinarily one would have thought that the policy makers, who I assume attended the same implementers meeting, would have flagged this ASAP to generate APPROPRIATE response immediately after the meeting. I also wonder why our media has not reported this or is it that there was no media representative from Nigeria at the Implementer¢s meeting???
Just to also add that this didn't come to me as a surprise because, just like Pastor JohnMark noted. I remember that Dr. Agwale presented this finding some 2 years ago during a NEPWHAN/INTERFAITH organized forum on 'New Prevention Technology and CSO involvement in their development processes', with technical support from NHVMAG. Dr. Ukpong and Dr. Agwale served as key resource persons. The report of the workshop captured this finding and was shared with FMOH and NACA which also participated in the forum. A brief on the workshop highlighting this was also posted on the e-forum. I also remember that following alarm raised by participating CSOs, a preliminary research findings on a study on treatment efficacy among PLWHA in Nigeria was publicly presented and posted on the e-forum by NIMR, showing only about 65% efficacy reasonable percentage of drug resistance (I think it was about 35%) among PLWHA on ARV. 2 years down the line, we are yet reminded
of this and there seems to be no evidence that we are seriously responding to this STATE OF EMERGENCY!
This is yet another challenge to all of us as advocates and Civil Society networks to demand that the right thing be done in good time to save lives! Time to ACT is NOW!

Honestly i have personally noted this trend as high resistance and failure of prescriptions, which usually effect quick change in ARV Regiment Drugs [ noted amongst few patients, i've paid 'presonal close-monitoring'] and this had given a big question mark to the overall gud success of the 'ARV/PEPFAR' program esp when the date of its discontinuation is approaching faster, except the In-Coming American President do something
In my opinion, i think observing the following could help to stem this dangerous trend:
[a] Constant & consistent checking of Patients VIRAL LOAD TESTING, to be general every 3months interval
[b] Creating more centers in all states for the checking of Viral load test, [Plateau state is luky to have- people go from Benue to have it done]
[c] GOVT AT THE STATES AND FEDERAL SHOULD COMMIT MORE FUNDS TO ESTABLISH MORE CENTERS, POSSIBLY 2 OF SUCH IN EACH STATES AND NOT ONLY IN THE CAPITAL
[d] More Training to Medical Personnel/Care Givers of the relevance to constantly check the Viral-Load Tests of thier Wards as CD-4 Count tests are important
[e] The involvement of Foriegn Donor Organization, to identify this problem, by providing fundings for On-The-Spot Monitorig of Privately owned ARV Centers
